Return on ad spend is measured against bookings and patient value from your PMS, not clicks, using your clinic's real average patient value rather than a default. Branded campaigns are recognised and discounted, because a search for your own clinic name is usually a returning patient. Where booking tracking is thin, the estimate falls back to click volume and is labelled as an estimate, never shown as a false zero.
